The goal is to guide residents, family members and/or caregivers through successful health and wellness transitions to achieve the optimal level of wellbeing and appropriate level of care. The Director of Resident Navigation provides relational services by building connections with the residents, gaining trust, and providing emotional and psychosocial support through the aging process.
Schedule: Full-time opportunity, Monday - Friday
Salary: $60,000 - $68,000/year

- Maintains awareness and promotes all internal services to promote resident retention.
- Coordinates new resident orientation.
- Evaluation of the physical, emotional and/or social needs of residents within community.
- Serves as resident liaison and advocate, including coordinating assessments, offering consultations, and providing assistance with coordination of both internal and external resources. Assist with clinical oversight, including blood pressure checks and vitals, offering wellness consultations, and guiding residents and families in coordination of health services (i.e., physician appointments, lab tests, x-rays, etc.).
- Interacts with the resident and family members when a change in the resident’s condition necessitates additional services or a physical move within the continuum of care. Assist with residents and family members by offering support in dealing emotionally and psychosocially with the aging process and illnesses.
- Works with health services team to coordinate services.
- Participates in Manager on Duty Rotation as requested by executive director.
- Participates in Staff recruitment, coaching staff, staff assignments, compliance, education and may assist in scheduling of staff.
- Bachelor’s degree preferred
- Minimum of three years’ experience in administration and management
- Senior living community experience preferred
