Salary: $55,000 to $60,000 annually based on education and experience

Organizational Relationship or Line of Authority: Responsible to Supervisor of Health Services, Superintendent of Schools, or Designee
Statement of Duties:
- Responsible for supporting the overall educational and social/emotional goals of the district.
- Social/Emotional support/processing for youth and families
- Ongoing communication with district principals, teachers and students in order to support the success of students and families
- Develop catalog of school and community based services for website and print materials
- Support families as they enter our school community and help connect them to resources so that they are better equipped when beginning school
- Visit local resources and network with community agencies
- Engage families and youth to facilitate or coordinate community and school based services
- Case management for families entering HPS with siblings across multiple schools
- Access to SNAP student health record database to collect and document referrals, resources, and support efforts and for progress monitoring
- Work collaboratively with nursing and mental behavioral health staff as well as with community mental health providers and partners
- Conduct a community mental health needs assessment and provide report to administration
- Collaborate on district policy and protocol related to mental health services and response
- Assist with SBIRT screenings
- Attend community based meetings
- Provide and/or assist with community trainings as needed in partnership with health/mental health staff
- Provide and/or assist with professional development for staff, as needed
- Home visits, as needed
- Such alternatives to the above qualifications as the Superintendent of Schools may find appropriate and acceptable
Qualifications:
- Bachelor's Degree in Nursing, Social Work, Psychology or other health related field – Required.
- Ability to work closely and harmoniously with HPS families, nurses, teachers, and administrators.
- Ability to conduct home visits and to write developmental histories.
- Knowledge of area mental health services available for HPS families and staff.
- Strong organizational skills.
- Knowledge of community resources
