Position Summary
Provides spiritual counseling and support for the Hawaii Care Choices and Palliative Care patients and families. Acts as liaison and educator between Hawaii Care Choices and community clergy and their organized faith community. Provides services utilizing principles of palliative care and the hospice philosophy. Directs and provides spiritual care for the Interdisciplinary Team.
Duties & Responsibilities
- Assesses the spiritual needs of the patients and their families. Initial contact and spiritual assessment shall be done within five (5) working days of admission. Provides direct spiritual counseling and support to the patient and family, in keeping with the patients’/families’ beliefs, as established in the Plan of Care.
- Conducts bereavement assessments of family members of the deceased and facilitates bereavement counseling as necessary.
- Participates in developing the interdisciplinary care plan, both as a consultant and as a provider of direct services. Actively participates in all interdisciplinary team conferences.
- Makes contact with clergy or appropriate representative of patients/families as indicated.
- Assists members of the interdisciplinary team to understand the significant spiritual concerns related to the patient’s/care giver’s responses to dying.
- Maintains documentation in patient records in accordance with company policy requirements (within one {1} business day of encounter).
- Participates in memorial/funeral services as requested for those families not affiliated with a church or those who request participation.
- Organizes, conducts, and/or participates in educational forums, seminars and/or in-service trainings concerning spiritual issues for community clergy and their faith organizations, staff, volunteers, health care professionals and the public at large.
- Provides training on spirituality at the volunteer training sessions, as requested by the supervisor.
- Completes other documentation such as time sheets, mileage, and other records as required.
- Liaison between the various community faith organizations and Hawaii Care Choices.
- Upholds and maintains the organization’s cost containment efforts without jeopardizing the quality of hospice care.
- Observes and promotes sound safety practices.
- Follows all company policies and procedures.
- Participates in meetings and committees as assigned or requested.
- Participates in program development and special projects.
Skill & Knowledge Requirements
- Ability to exercise independent sound judgment in decision-making, planning, and provision of quality spiritual care and education.
- Possession of critical assessment and thinking skills.
- Ability to provide unsupervised spiritual services in private residences.
- Self-motivated and able to work independently, as well as in cooperation with the interdisciplinary team.
- Ability to be open, flexible, and ecumenical.
- Ability to embrace people from different ethnicities, beliefs, and faith traditions.
- Interest and desire to give hospice care with a special sensitivity to the needs of the dying.
- Ability to prepare reports, statistics, and educational presentations.
- Competent with Windows, Microsoft Office and Power Point.
Education & Experience Requirements
- Bachelors’ degree or equivalent with experience in / and a desire to provide spiritual care.
- Adequate educational and experiential levels to meet the needs of the patients, care givers, and regulations established by state and federal laws.
- Prefer a person with experience providing bereavement and spiritual care in a hospice type setting.
Hours
Provide PTO/shortage Spiritual Counselor coverage as assigned. May stagger weekdays and weekends, and with occasional evening and holiday hours.
Pay $29.00 - $32.00 / hourly
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
Pay: $29.00 - $32.00 per hour
