Haymarket Center is the largest not-for-profit community-based adult detoxification, residential, and outpatient substance abuse treatment facility in Chicago. Haymarket Center has continued to grow into a comprehensive alcohol and other drug treatment organization, licensed by the state of Illinois, which receives funding from the private sector, as well as city, county, state and federal agencies. The treatment programs are accredited by the Commission on Accreditation for Rehabilitative Facilities (CARF).
The Assessor is responsible for the completion of the Global Appraisal of Individual Needs assessment. The Assessor must possess thorough understanding of ASAM, Substance Use and Mental Health Disorders.
ESSENTIAL FUNCTIONS:
•Must complete at least three assessments per day .
•Must possess strong writing skills
•Complete domestic violence assessments when appropriate .
•Place patients in appropriate level of care .
•Must have strong computer skills
•The candidate should be able to manage their time in a way that would ensure quality and productivity measures.
•Triage with other professionals to ensure comprehensive quality care for the patient.
•Work with admissions counselor to schedule patient's admission.
•Coordinate patient's attendance to pre-treatment groups .
•Other duties as required for effective team work or as assigned by Supervisor.
Requirements
EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:
•High School diploma or equivalent.
•Some college preferred, particularly in substance abuse or related field
•Must be GAIN certified within 90 days of hire .
•Must be certified by IAODAPCA at CADC level.
•Must have knowledge of current ASAM criteria.
•Good Verbal and written skills required.
•Bilingual or sign language beneficial.
•Assessment skills required.
•Previous assessment experience beneficial. Must be able to form a diagnostic impression by using bio-psych-socials and other assessment tools to determine the level of care needed by the patient, including patient strengths, problem areas, and the areas of clinical and medical concerns, impressions, recommendations, and developing a diagnostic summary.
