Description
This position will ensure Haymarket Center client files are in compliance with the agency’s policies, procedures, and standards to meet external regulatory requirements.
ESSENTIAL FUNCTIONS:
- Review client charts to ensure that all intake procedures have been followed, and that the admission criteria have been met based on the clinical diagnosis and supporting documentation.
- Review treatment plan development to make certain that the clinical needs of the client are being documented during the treatment stay.
- Monitor client charts to ensure that the progress in meeting the objectives and goals of client treatment plans is being documented. Validates that continuing stay criteria are being met.
- Confirms that discharge criteria and continuing/aftercare plans are in place and consistent.
- Documents client chart reviews in a timely manner, and recommends corrective action needed. Seeks supervision when necessary.
- Assists in the training of client charting for the Central Intake and Clinical Staff.
- Assists in the gathering of data for client treatment outcome measurements. Performs other job duties as assigned.
- Producing and sharing reports on QI/QA to support decision-making and oversight by key management staff and by the governing board regarding the provision of health center services.
- Conduct personnel file review to ensure proper credentialing and privileges of clinical staff, including trainings and certifications in basic life support.
- Monitoring QI/QA Outcomes
- Updating QI/QA operating procedures and procedures when needed.
Requirements
- High school diploma or equivalent.
- Certified Alcohol and Drug Counselor (CADC)
- Licensed Counselor (preferred)
- Certified Content Expert (CCE) of Certified Healthcare Compliance (CHC) (Preferred)
- Familiarity with Administrative Code Part 132, 140, 2060, 2090, and CARF Behavioral Health Standards and JCAHO.
- Familiar with Health Center Program Standards as set forth by HRSA.
- At least 2 years of record keeping experience with a substance abuse facility or a combination of skills and qualification.
- Must be detailed oriented, demonstrate proficient written and verbal skills.
- Relate well with others.
- Able to work independently with minimal supervision.
