Description
Peer Specialist is trained to incorporate their unique personal experience in their recovery with a distinct knowledge base and human service skills. This combination of experience and training allows the PS professional to facilitate recovery and build the resilience of persons in recovery.
ESSENTIAL FUNCTIONS:
Assist individuals in becoming involved in their own recovery process so they might develop personalized action plans for their own person, emotional, physical and social health;
Provide case management, referral and referent contact services to patients and documents same in patient record;
Assist the participants with connecting to mainstream benefits and suitable housing;
Facilitate peer groups to help reduce recidivism due to anti-social peer network groups;
Assists residents to achieve the goals of mutually agreed upon care plans for substance rehabilitation service needs and aftercare.
Completes required forms pertaining to patient case management needs within the program;
Finds and develops program use needed resources including visiting and inspecting agencies and attending network meetings.
Completes baseline and discharges in accordance with IDHS/SUPR grant requirements;
Assists residents to link with needed family and community resources as coordinator and advocate;
Attend patient staffing and/or interventions;
Seeks appropriate supervision requiring clinical or programmatic decisions;
Other duties as assigned.
Requirements
EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:
•High School Diploma or equivalent;
•Knowledge in substance abuse or related field;
•Familiar with case management services and able to apply them to recovery management;
•Good verbal and communication skills. Proficiency in reading and writing;
•Good computer literacy is mandatory;
•Able to work effectively on a team.
