Position Summary: Under the supervision and cooperation of the Director, the supervisor oversees the daily processes of the nursing unit to ensure that safe, evidence based care is provided. The supervisor contributes to the overall corporate objectives by providing leadership to the unit, assisting with financial management and maintaining policies. The supervisor serves as the Hospice Educator and will assess, implement and evaluate the associate’s education needs to promote quality patient outcomes. The Hospice Supervisor implements processes to support evidence based practice in the delivery of quality health care enhancing patient satisfaction and quality goals. Responsible for providing care, comfort, and support to patients navigating the difficult time at the end of their lives. Develop plans for individual patients to ensure their needs are met, perform regular visits, and communicate with care team and patients support system.
Responsibilities:
- Provides positive leadership by an environment of teamwork, open communication and customer service.
- Responsible for human resources processes including interviewing and orientating staff, ongoing performance management and timekeeping/scheduling and required documentation.
- Assists with financial management by developing and maintaining budget, strives to keep operations within benchmarks and communicates variances.
- Ensures the compliance of policies and work instructions with regulatory standards to improve the overall operations of the facility.
- Maintains familiarity with all relevant emergency management policies and procedures.
- Collaborates with the Director of Clinical Care and Medical Director.
- Assists with the maintenance schedules on equipment and ensures that equipment is safe for patients and personnel.
- Participates in and utilizes evidence based research for improving staff education activities, nursing practice and staff outcomes.
- Collaborating with physicians and other professionals to plan care.
- Collects and assesses initial and ongoing data about the health status of the patient.
- Initiates and or revises an individual plan of care that includes immediate and long-term outcomes for the patient/family and reflects the plan of the entire health care team.
- Carries out interventions safely and in compliance with policy to facilitate achievement of expected outcomes.
- Provides patient/family education by explaining procedures, medications and giving instructions at a level the patient/family can understand; addresses patient's questions and concerns; verifies patient information and consent when indicated.
- Documents and communicates all required components of patient care.
- Identifies physical symptom changes and provides appropriate treatment.
- Demonstrates established techniques of safe administration of medication according to hospice poliy and procedures.
- Demonstrates competency in a variety of therapeutic/diagnostic interventions including but not limited to incision/wound care, medication administration, activities of daily living and catheterization.
- Demonstrates ability to adequately assess and reassess pain.
- Utilizes appropriate pain management techniques.
- Promotes, advocate, and collaborates to protect the health, safety, and rights of each patient.
- Provides patient/family education through assessment of patient/family learning readiness, needs and ability, and evaluates effectiveness of teaching.
- Provides community education on Hospice
- Uses standards of care, hospital policies/procedures and regulatory guidelines to guide practice.
- Functions effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care.
- Documents ongoing status, interventions, patient response and outcomes in accurate, timely manner.
- Seeks out learning opportunities and continuing education.
- Effective use of time, equipment, supplies to provides high-quality, cost-effective patient care.
- Through and confidential maintenance of patient charts and records
- Uses information technology to communicate, manage knowledge and mitigate error and support decision making.
- Reviews and audits Hospice quality measures, patient satisfaction, Hospice documentation/claims
Qualifications:
Required:
- Current Kansas Registered Nurse License Graduate of an accredited school of nursing.
- Bachelor of Science in Nursing (BSN)
- Knowledge of general nursing and practice. Current registered nurse licensure in the State of Kansas or has work permit from the State Board of Nursing.
- BLS certification required or must complete BLS certification with in the first two weeks of employment.
- Additional certification may be required for certain patient care settings or populations.
Preferred:
- Related supervisory experience
- Masters degree
Patient Interaction: Periodic
Infection Control: Initial and Ongoing training in dealing with infection control. Trainings could include but are not limited to, blood borne pathogens, bodily fluids and bio hazardous materials as it applies to your daily work environment.
