Job Summary
We are seeking a compassionate and dedicated Case Manager to join our dynamic team, working closely with physicians and mental health therapists. This role is pivotal in assessing the requirements of individuals facing crises and crafting personalized care plans. As a Case Manager, you will manage a caseload of adults diagnosed with serious and persistent mental disorders, playing a critical role in their journey toward recovery and stability.
Key Responsibilities:
-Provide face-to-face crisis contact with individuals in need.
-Available to go to emergency rooms 24/7 according to on-call schedule
- Collaborate effectively with a multidisciplinary team to provide comprehensive support to individuals in crisis.
- Swiftly respond to crisis situations within the community, providing immediate support and intervention as needed.
- Develop, implement, and monitor effective treatment and service plans tailored to each individual's needs, facilitating their adherence and compliance.
- Serve as an active and empathetic member of an interdisciplinary treatment team, ensuring a cohesive and integrated approach to mental health care.

Qualifications
