Director of Social Work
Our home is look for a qualified, hard-working and customer service driven Social Worker with long-term care experience to join the team.
Director of Social Work Responsibilities Include :
- Maintaining and / or improving resident’s ability to control everyday physical needs and mental / psychosocial needs
- Identifying individual social and emotional needs
- Developing and maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
- Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
Qualifications / Requirements :
Salary Range : Up to $110,000 a year (Based on experience)
