Social Worker

Our ideal candidate is a compassionate and dedicated professional who is committed to providing exceptional care and support to our residents.

Key Responsibilities:

• Maintain and improve resident ability to control everyday physical needs and mental/psychosocial needs.
• Identify individual social and emotional needs.
• Develop and maintain individualized care plans in collaboration with interdisciplinary teams.
• Participate in planning of resident admission, community re-entry, or transfer to another facility.
• Collaborate with finance team to ensure resident education on financial matters and completion of necessary documents during stay.
• Work closely with residents on discharge to secure necessary DME or home care for safe transition back to the community.

Requirements:

• BSW or MSW preferred; Bachelors degree in related field also accepted.
• At least 3 years experience in Social Work required.
• Able to work flexible schedule.
• PCC experience a plus.
