Job Overview
We are seeking an experienced Home Health RN to join our team. As a key member of our interdisciplinary team, you will be responsible for providing coordinated skilled nursing care to patients of all ages, while working collaboratively with physicians and other healthcare professionals to establish and achieve patient goals.
Key Responsibilities:
- Assess home care patients identifying physical, psychosocial, and environmental needs.
- Develop and implement individualized plans of care in collaboration with the interdisciplinary team.
- Provide ongoing education and support to patients and caregivers.
Requirements:
- Graduate of an accredited school of professional nursing.
- Minimum of one or more years of home health, public nursing, or acute hospital nursing experience.
- Familiarity with Medicare home health regulations, documentation requirements, ICD-10 coding, and PPS.
