Job Description
We have an exciting opportunity for a Registered Nurse (RN) Case Manager to join HCA Florida Lake Monroe Hospital, part of the nation's leading provider of healthcare services, HCA Healthcare.
Key Responsibilities
- Provide program orientation to patients, families, and caregivers, including case manager role, rehabilitation philosophy, and continued stay and discharge criteria.
- Coordinate patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure implementation of patient treatment plans that support strengths, abilities, needs, and preferences, facilitating patient involvement throughout the rehabilitation process.
- Document findings of Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of patients, families, and caregivers.
- Promote consistent, positive patient interactions advancing the agenda of unparalleled patient service.
Requirements
- Florida RN license and/or approved multistate RN license required; compact license holders have 60 days to obtain the Florida license after relocation.
- BLS-AHA Provider issued card required within 30 days of start.
- Completion of RN Diploma.
- Minimum of three years recent acute clinical practice or related healthcare experience.
