The Patient Educator - Dietician educates patients with various nutritional needs, including diabetes, bariatric, metabolic and other needs, including pediatric and adolescent patients. Includes the patients, family members, and significant others in the education and care processes.

Job Summary And Qualifications
The Patient Educator - Dietician educates patients with various nutritional needs, including diabetes, bariatric, metabolic and other needs, including pediatric and adolescent patients. Includes the patients, family members, and significant others in the education and care processes.
POSITION RESPONSIBILITY
- Assists in developing and teaching nutrition education programs on an ongoing basis for both inpatients and outpatients.
- Serves as liaison between the consulting physician and staff regarding the education of the patient with nutritional needs.
- Participates and promotes the interdisciplinary process regarding the patient status, and plans of care.
- Assists and participates in the ADA Recognition process and Advisory Council.
- Collaborates with members of the health-care team to identify patient care needs and to assist with planning, implementation, and evaluation of comprehensive care of the patient and family.
- Conducts patient education classes and individual consults, including follow-up.
- Facilitates, participates, and attends quarterly community lectures.
- Assists with maintaining diabetes supplies, educational materials, and products.
- Handles phone inquiries as appropriate.
- Lectures to various community groups.
- Substitutes for others on the patient education team as needed.
- Maintains continuing education in current nutritional care, diabetes management and other programs.
- Assists with maintaining compliance of Joint Commission standards regarding patient and family education.
- Provides support for and assistance with hospital staff education and development, (such as continuing education and in-service education), and other patient education activities.
- Provides patient and family teaching using age appropriate resources & guidelines. Age groups served to include children, adolescents, adults, and geriatrics.
- Participates and attends community education events to promote awareness
- Shares and promotes the hospital’s Mission, Vision, and Values: supports the Ethics and Compliance Program.
- A minimum of two year's experience working in a clinical setting, preferably with patients having obesity, metabolic nutrition needs and diabetes.
- At least one-year experience in education, preferred.
- Basic Cardiac Life Support
- Licensed Dietician
- Associate Degree, or Bachelors Degree, or Masters Degree
