About the Role
The PRN Home Health Physical Therapist provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team.
Key Responsibilities
- Evaluate patient conditions and develop individualized care plans focusing on goal achievement, patient engagement, and positive patient experience.
- Provide or direct a majority of the patient's physical therapy to support patients throughout the continuum of care.
Requirements
- Licensed Physical Therapist.
- Drivers License.
- Bachelors Degree.
