Job Summary
The Physical Therapist provides high-quality patient-centered care by managing the complete process of therapeutic care for an assigned group of patients.
- Assess and evaluate therapeutic/rehabilitative/functional status.
- Develop and implement individualized care plans focusing on goal achievement and patient engagement.
- Provide purposeful and skilled therapeutic care at patients' homes.
- Instruct patients/families/caregivers in the use of therapies and exercises.
- Prioritize caseload and determine daily staffing needs.
Requirements
- Licensed Physical Therapist.
