Job Description
- Assess patients' physical, psychosocial, and environmental needs to identify areas for improvement.
- Develop and implement comprehensive care plans in collaboration with the healthcare team.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on self-care.
- Supervise and direct home health aides and LPNs/LVNs to ensure quality and continuity of services.
