Position Overview : We seek a self-motivated and energetic Care Coordinator to join our team. In this salaried role, you will support caregivers, ensure excellent care delivery, and uphold the professional standards of Homewatch CareGivers. Reporting directly to the Operations Manager, you will be pivotal in facilitating successful relationships between caregivers and clients, leading to positive care outcomes.

Key Responsibilities :

  • Client Onboarding : Manage new client onboarding, ensuring all aspects are set up correctly on the first day of care in accordance with company guidelines.
  • Field Visits : Conduct field visits every 7-10 days for all shifts, complete documentation, and provide caregivers with necessary supplies.
  • Care Plan Management : Review and update home safety assessments and Individual Care Plans (ICPs), ensuring accuracy in medication reminders, fall risk assessments, mobility needs, and daily routines. Document all changes in KanTime and communicate updates to the care team.
  • Caregiver Support : Provide coaching and encouragement to caregivers to enhance performance and client relationships. Escalate disciplinary issues to the Operations Manager when necessary.
  • Team Collaboration : Work closely with the Operations Manager, scheduler, and office staff to match caregivers with clients for optimal relationships.
  • Communication : Ensure clear, timely communication with all team members to prevent process breakdowns. Respond promptly to phone calls and messages.
  • Caregiver Transitions : Facilitate smooth transitions by introducing new caregivers to clients, reviewing the ICP, and addressing any questions.
  • Compliance : Maintain confidentiality in compliance with HIPAA and agency policies. Report any incidents involving caregiver injuries or critical incidents to the Operations Manager immediately.
  • Care Coverage : Provide care coverage if a caregiver cancels or no-shows a shift, including handling last-minute situations.
  • Documentation : Review and update client and caregiver daily journals.
  • On-Call Participation : Participate in on-call rotations as required.
  • Professionalism : Model professional behavior, dress, and adherence to company policies at all times.
  • Qualifications :

  • Experience : Minimum of 10 years of caregiving experience preferred; previous healthcare experience is a plus.
  • Skills : Strong verbal and written communication skills; exceptional organizational, multitasking, problem-solving, and de-escalation abilities.
  • Technical Proficiency : Proficiency in Microsoft Office Suite and adaptability to company software systems.
  • Physical Requirements : Ability to work 40 hours per week; capable of bending, climbing, stooping, and standing for up to five hours a day; able to lift 20-30 pounds as required by the role.
