What are the responsibilities and job description for the Community Health Worker - Financial Counselor position at HCC?
Are you looking to make a positive impact on families that live in rural communities? The Community Health Worker – Financial Counselor (CHW-FC) is responsible for helping patients, individuals, and their families navigate and access community services and other social and financial resources. The CHW will work on activities that will promote, maintain, and improve health outcomes; provide social support; informal counseling, and be an advocate for patients and their families. As a financial counselor, they will be the liaison between the clinics and the billing office, providing support to patients to ensure they provide the proper documentation for claims filing, income verification and applications for program enrollments such as Medicaid and Slide Fee Scale.

Pay: $15.00 - $17.00 per hour
