Chronic Care Management - Case Manager

We are seeking a compassionate and Case Manager to join our team at HCLS Consulting. *MUST HAVE* an educational background in Nursing, Medicine related science field since the role requires managing the chronic conditions of patients. We work with medical practices in NY and California. In this role, you will be responsible for remotely managing and coordinating the care of patients with complex medical needs. This position is responsible for the coordination of care for our patients, as well as managing their chronic conditions - typically Hypertension and Diabetes.

Duties include but are not limited to:

You will work independently to develop and implement individualized care plans, and will have support to facilitate access to community resources, and provide ongoing support to ensure positive health outcomes.

The ideal candidate will have experience working in a healthcare setting, be proactive and have the willingness to learn, be a self-starter, and possess excellent computer, communication and problem-solving skills.

Requirement:

- Background in Nursing, Pharmacology, Medicine, Nutrition or a related sciences is a REQUIREMENT.
- Tech savvy
- Proficient in MS Office
- Efficient Communication

Compensation:
This is a contractual position. The compensation is based on your caseload - 150 to 200 patients a month if working full time.
