Our Client, a reputable medical organization in the Bronx is seeking a Director of Social Services
Duties and Responsibilities :
- Ensure that all Social Work documentation is completed in accordance with facility policy and procedure as applicable
- Coordination of Interdisciplinary Discharge Planning (Knowledge of community resources)
- Demonstrate compliance with government and accrediting agency standards and regulations pertaining to Social Work
- Interface with facility departments and outside agencies in order to provide a complete plan of care and to facilitate services.
- Liaison between family and other interdisciplinary team members
