Role and Responsibilities
Administer practical nursing care to patients in accordance with physicians orders under the direction and supervision of a Registered Nurse.
Essential Functions
Under the direct supervision of an RN Case Manager, and within the scope of practice for a Licensed Practical Nurse :
Provide health education according to the teaching plan established by the RN.
Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.
Observe signs and symptoms, and report patients reactions to treatments including drugs and changes in the patients physical or emotional condition, to the physician and RN Case Manager.
Document any services rendered to the patient, changes noted in the patients condition, and / or family and home situations.
Document communication with RN Case Manager and physician.
Other duties as assigned.
