Role and Responsibilities
The Manager, Care Management manages the day-to-day functions of a designated Health Home Team. Provides support including care planning direction to Health Home Care Managers, conducting field supervisions and assisting as needed with mentoring and continuing education. Monitors Team performance and client trending for optimal outcomes / results.
Essential Functions
- Plan, coordinate and evaluate the departmental objectives in accordance with corporate strategies in collaboration with the Executive Director of Care Management, and aligns execution of Team goals.
- Manage and monitor assigned staff performance including: orienting new staff, performing competency evaluations, reviewing case documentation, and case assignments for quality and completeness.
- Evaluate efficiencies and productivity of staff members and the team as a whole, by looking for trends in reports including weekly reviews of productivity, utilization, documentation, caseload size, and client acuity. Communication with Executive Director of Care Management regarding issues, trends and recommended follow-up action.
- Serve as a member of the client case management team to intra-departmentally address client and or procedural needs.
- Work within the team to recognize areas for continuous process improvement including any HIT improvements.
- Proficient in use of multiple software systems and is able to provide correction when needed regarding documentation in any one of the EMR’s as needed, including the Lead Health Home systems, HCRs Database and the HCS site for UAS MH Assessments.
- Communicate effectively with all managers/departments within HCR Care Management and HCR Home Care.
- Assist HHCM’s by providing guidance on client issues, community and financial resources.
- Hold routine case conferences with assigned staff to review random sample of client records based on audit requirements.
- Address and document HHCM performance issues on a timely basis, in a clear and concise manner.
- Conduct periodic performance updates with team members, including annual performance reviews.
- Investigate and address concerns regarding department/staff received from clients and families.
- Perform client visits/enrollments as needed.
- Other duties as assigned.

Education Requirements
- Master’s Degree in any Human Service Field AND 2 years of experience providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or Substance Abuse OR Linking persons who have been diagnosed with mental disabilities, developmental disabilities, alcoholism or Substance Abuse to a broad range of services essential to successfully living in a community setting.
- Bachelor’s Degree AND 4 years of experience providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or Substance Abuse; OR Linking persons who have been diagnosed with mental disabilities, developmental disabilities, alcoholism or Substance Abuse to a broad range of services essential to successfully living in a community setting.
Qualifications and Requirements
- One to two (1-2) years supervisory or management experience in a Care Management (Health Home) setting.
Work Environment
The Manager, Care Management is primarily in an office setting and may be exposed to outdoor conditions.
The working conditions are classified as sedentary work:
- Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
