Job Overview
As a Case Manager, you will be responsible for providing comprehensive case management services to individuals and families in need. This role involves assessing clients' needs, developing care plans, and coordinating services to ensure clients receive the support they require.
Responsibilities
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide crisis intervention and support to clients facing difficult situations
- Coordinate with external agencies and service providers to access necessary resources for clients
- Monitor client progress and adjust care plans as needed
- Supervise and mentor clients in areas such as workforce development, childcare, and behavioral health
- Utilize motivational interviewing techniques to empower clients to make positive changes in their lives
- Familiarity with Medicare, Child Welfare, Section 8 housing, and other relevant programs
Experience
- Previous experience in social work or related field preferred
- Strong understanding of child welfare systems and behavioral health services
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Knowledge of community resources and services for client referrals

Pay: $25.00 - $27.00 per hour
