What are the responsibilities and job description for the Community Integration Counselor (CIC) position at Head Injury Association?
Head Injury Association is seeking a Community Integration Counseling (CIC) to our growing team. CIC’s offer individualized counseling service directly to our waiver participants, at times the participants and their families, to learn to effectively manage the emotional difficulties associated with adjusting to life after a traumatic brain injury. These services are conducted in home to our individuals and offers counseling service provided to a participant coping with the need to revise long term expectations, his/her changing roles and the impact on him/her, family members and informal supports.
Job Responsibilities:
- Implementing the individualized behavioral plan and tracking its effectiveness as assessed through supervisory review of documentation.
- Adherence to confidentiality rules will be consistent with HIPAA regulations and accepted professional standards as assessed through supervisory observation and review of documentation.
- Maintain open communication with the waiver participant’s Service Coordinator and other service providers and team members in an effort to enhance the delivery of services to the participant.
- Contemporaneous entry of daily notes into MediSked in accordance with agency policy and in adherence with regulatory agencies as evidenced by supervisory review of documentation.
- Participate in team meetings are scheduled periodically to address participant’s needs and services; the CIC provider must attend the meetings.
- The CIC is responsible for all billing and record-keeping procedures for CIC Services provided to their assigned participants as assessed through supervisory review of documentation.
- The CIC must communicate with their supervisor regarding the behavioral plans, the participant’s reaction to it and any changes or suggestions as deemed necessary as assessed through supervisory observation and/or review of documentation
- Keeps appropriate staff informed of relevant information and incidents in the unit including any behavior believed to be an emotional, physical or psychological disturbance as evidenced by supervisory review of documentation and observation.
- Demonstrates knowledge of TBI as evidenced by communication with clients, log entries, staff and or team meeting, formal education or HR’s training records and observations or interactions with consumers.
Job Requirements:
MUST BE ONE OF FOLLOWING:
- Licensed Psychiatrist (Licensed by NYS Education Department)
- Licensed Psychologist (Licensed by NYS Education Dept)
- Licensed Masters of Social Work (LMSW – licensed by the NYS Education Dept)
- Licensed Clinical Social Worker (LCSW – Licensed by the NYS Education Dept)
- Certified Rehabilitation Counselor (CRC) – Certified as a CRC by the Commission on Rehabilitation Counselor Certification
And
- Have Valid NYS Driver’s License
- Two years’ experience providing adjusted related counseling to individuals with traumatic brain injuries and their families
