Description
We are a leading Human Services agency located on Long Island. Our Agency is in need of a few experienced, highly motivated and talented individuals that want to grow with us in this part-time position.
ILST services may include assessment, training, supervision of, assistance to an individual with issues related to self-care, medication management, task completion, communications skills, interpersonal skills, socialization, sensory/motor skills, mobility, community transportation skills, money management, problem solving, pre-vocational and skills to maintain a household.
The primary goal is to improve the ability of the participant to live as independently as possible in the community.
**Day Shifts available**
Requirements
- One (1) to Five (5) years experience (base on degree) completing assessments of ADL/ADL's developing a comprehensive service plan, and teaching individuals with Traumatic Brain Injury, disabilities, and/or seniors to become more functionally independent
- Associate's Degree and (5) years of experience
- Bachelor's Degree and (3) years of experience
- Master of Social Work or Psychology Degree and one (1) year of experience
- Valid New York State Driver's License
