Overview:
Camino a Casa, a subsidiary of Heading Home, Inc., seeks a talented, hard-working, and eager candidate to join our housing and stabilization team to provide housing placement and stabilization support to homeless and at-risk households. The Stabilization specialist will assist households to secure and maintain permanent housing and develop/implement an individualized housing stabilization plan to meet their own identified goals and remain stably housed.
Responsibilities:
- Provide Case/Housing management services to families transitioning to their own housing.
- Develop, input, and revise (as necessary) stabilization plan.
- Contact Stabilization clients at least once a month to review and update Stabilization Plan.
- Meet with families face to face quarterly as well as keep telephone monthly contact log.
- When necessary, identify other potential permanent housing opportunities i.e., public housing, low-income housing, lotteries, fair market rent
- Assist families in overcoming obstacles and barriers preventing permanent housing stability
- Maintain written documentation on housing and overall progress as well as enter monthly reports and case notes into State’s ETO touchpoint system.
- Coordinate stabilization services to families placed in housing to ensure safe, sanitary, affordable housing stability.
- Conduct regular home visits (when necessary) with participants to support them in maintaining housing stability.
- Assist families and /or individuals to connect with community services and make appropriate resource referrals
- Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements.
- Accompany participants when necessary to support them in obtaining housing and other community services.
- Establish landlord/tenant mediation and advocacy (when necessary): Follow up monthly with landlord on tenancy
- Knowledge of mandate reporting guidelines of child neglect and abuse under 51A requirements
- Establish a respectful relationship with clients
- Other duties as requested by the program director or supervisors.
- Ensure compliance with program/department/agency requirements
- Perform other duties as assigned.
