Job Title: Director of Community Support Program for Homeless Individuals (CSP-HI)
Reports To: Senior Director of Permanent Supportive Housing and Clinical Programs

Overview:
The Community Support Program for Homeless Individuals (CSP-HI) is a specialized high touch case management program designed to assist individuals or families experiencing homelessness by providing support before, during, and after transitioning into permanent housing, with a focus on those who frequently utilize medical or behavioral health services through MassHealth. Heading Home contracts with Eliot Community Human Services for third-party billing to MassHealth for the CSP-HI program.
The Director of CSP-HI is responsible for overall program operations including supervision of all CSP-HI program staff, monthly billing, program growth and best practice policy implementation. The Director of CSP-HI will provide ongoing regular supervision to case managers that support individuals managing their own physical health and medical service needs. The Director of CSP-HI will oversee and monitor all needs assessments, action plans, crisis plans, billing, CSP-HI service notes and referral services.
The Director of CSP-HI will play a key role in the organization’s leadership team by working closely with the Senior Director of PSH and Clinical Services on program design, structure of service, daily management and billing for the program. The Director of CSP-HI will ensure that the Heading Home CSP-HI program meets service delivery goals and contractual obligations, making necessary modifications to program design and organizational structure, as well as shaping and implementing program expansion and new programs. Heading Home programs operate using a trauma informed lens and using evidence-based models like Motivational Interviewing and Mobility Mentoring. The Director is expected to utilize these modalities and ensure that all staff are trained and provided with services using those models.
RESPONSIBILITIES / Essential Job Functions:
- Provide direct supervision to the case management department.
- In collaboration with other members of the team, determines and implements appropriate policies and procedures to ensure smooth and effective service delivery to individuals and compliance with regulations, contracts, and Heading Home policies.
- Oversee the delivery of CSP-HI client services, evaluate and assess program impact, recommend program/policy changes, and direct program shifts and improvements.
- Work closely with Eliot Community Human Services for monthly third-party billing to MassHealth.
- Work closely with Eliot Community Human Services to ensure fidelity to model of all CSP-HI program regulations.
- Ensure staff are professionally trained in deliverable services, referrals/intake/screening procedures, action/crisis plans and CSP-HI case notes. Preferred knowledge of Clarity and ETO systems.
- Ability to network with hospitals, health centers, physical health providers and other medical services geared towards individuals and families.
- Advise on, and at times directly manage, challenging client situations in partnership with other housing services staff members.
- Work closely with the Director of Supportive Housing to ensure case management resources and Heading Home Supportive Housing policies are uniform across both case management teams.
- Represents Heading Home at external meetings with housing providers.
- Demonstrate organizational written and verbal communication skills.
- Collaborate with the Senior Director of Housing and Data Team to develop and implement the tracking of process and outcome data for the Housing services portfolio of programs, ensuring that programming is following all local, state, and federal laws as well as with all sources of government funding.
- Serve as a key member of the leadership team, collaborating with colleagues in the development and monitoring of CSP-HI budgets and ensuring compliance with agency's purchasing and procurement policies.
- Adheres to all agency and program policies, procedures and rules.
- Performs all other reasonable related duties as assigned by the Senior Director of Permanent Supportive Housing and Clinical Programs.
- Proven proficiencies to manage priorities in a face moving setting.

Work Environment
- Office space within office building; prolonged sitting at a desk and working on a computer. Office is open to the public and visitors only by appointment. Position requires a minimum of four days onsite. Office is climate controlled with heat and air conditioning.
- Must be able to occasionally ascend/descend stairs up to three or more sets, occasionally move equipment weighing up to 25 lbs.
- Hours: Monday-Friday, 8:30am to 5:00pm 40hrs-week. 1 remote day, after 30-days of employment.
