Job Summary:
We are seeking an experienced and compassionate Wound Care RN Case Manager to join our team. This role involves delivering specialized care to patients with chronic and acute wounds, coordinating care plans, and collaborating with a multidisciplinary team to achieve optimal healing outcomes.
Responsibilities:
- Assess and manage patients with complex wound care needs, including pressure ulcers, diabetic ulcers, surgical wounds, and other chronic conditions.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Provide patient and caregiver education on wound care, prevention, and maintenance.
- Coordinate with insurance providers to ensure authorization of treatments and services.
- Monitor patient progress, document care accurately, and adjust treatment plans as necessary.
- Participate in team meetings and case conferences to discuss patient care and outcomes.
- Ensure compliance with all clinical protocols, safety regulations, and infection control policies.
Qualifications:
- Current and valid RN license in Florida.
- Minimum of 2 years of nursing experience, with at least 1 year in wound care or case management.
- Certification in Wound Care (e.g., WOCN, CWCN) preferred.
- Strong knowledge of wound care techniques, products, and treatment protocols.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work collaboratively in a team-oriented environment.
