The Referral Coordinator is responsible for processing medical, dental, and specialty referrals; as well as, PAP applications for patients of HHM Health.
Here's a sneak peek at what you'll do:
- Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety
- Update providers on issues or concerns regarding referrals and communicate directly with patients and providers.
- Works with insurance companies and care providers to share essential patient information and assists primarily in scheduling appointments and managing health insurance issues
- Connect reports to patient charts and notify provider of results
- Communicates with patients to complete referral process
- Work and track outgoing referrals by urgency and requested timeline
- Share your expertise and best practices to help us become the provider of choice
What you need to succeed
To become one of our CARES-givers, you will have excellent patient engagement, strong communication skills and an eye for continuous improvement. You will also have the following:
- High School Diploma or equivalent
- Prior experience as a Referral Coordinator or similar in a healthcare setting
- Experience in community health centers and/or non-profits is preferred.
- Detail-oriented and ability to multi-task in a high volume environment
- Strong written and verbal communication skills
- Experience serving multicultural and multilingual patient populations
- Experience with eClinicalWorks or similar and MS Office Suite skills
- Bilingual in Spanish, Pashto, Burmese, Rohingya or any other languages
- Have a servant heart with the desire to make a positive impact
