Job Summary
We are seeking a dedicated and skilled Community Integration Support Specialist to join our team. The ideal candidate will play a crucial role in ensuring the seamless integration of various collaborations with healthcare services and develop and implement strategies to enhance patient care. This position requires a compassionate individual who is well-versed in patient care practices, including creating support plans that promotes a persons recovery or needs to sustains a persons living needs.
The Community Integration Support Specialist staff will provide individualized evaluations and interventions based on the needs of the client. Case management consists of intake/assessment, plan of care development, coordination, referral and related activities, advocacy, monitoring and follow-up, and evaluation. Treatment services are designed to retain or improve functional abilities which have been negatively impacted by the effects of cognitive or functional impairment and are focused on behavior modification and management, social development, and acquisition and retention of developmentally appropriate skills.
Education requirements:
Must have a bachelors degree in Social Work, Psychology or other related degree and be MHRT/C certified which is an individual who has received Mental Health Rehabilitation Technician/Community certification from DHHS.
Responsibilities
A Community Support Provider furnishing Community Integration Services must be a certified MHRT/C and perform the following:
- Identifies the medical, social, residential, educational, vocational, emotional, and other related needs of the member;
- Performs a psychosocial assessment, including history of trauma and abuse, history of substance use, general health, medication needs, self-care potential, general capabilities, available support systems, living situation, employment status and skills, training needs, and other relevant capabilities and needs;
- Facilitate formal and informal opportunities for career exploration during services.
- Develops an ISP and maintain clinical files that is necessary to meet their individual goals;.
- Coordinates referrals and advocates access by the member to the service(s) and natural support(s) identified in his or her Individual Support Plan;
- Provide therapeutic support to develop and maintain community support, skills of daily living, symptom management, stress management, assertiveness, conflict management, and social community integration;
- Develop and provide groups consisting of education, skill development, peer and family support, recovery and wellness;
- Attend regularly scheduled supervision and staff meetings by seeking consultation and supervision as needed and appropriate;
- Ensure all client services being provided comply with all agency, state and federal guidelines and regulations.
- Need strong interpersonal skills to effectively communicate with patients, families, and healthcare professionals.
- A compassionate approach to patient care, demonstrating empathy and understanding for diverse needs.
