We are seeking a dedicated and organized Home Health Intake Coordinator to join our team. As the Intake Coordinator, you will play a crucial role in the smooth functioning of our home health services by managing the intake process for new patients. This includes gathering essential information, coordinating with healthcare professionals, and ensuring a seamless transition for patients into our care.
Responsibilities:
- Receive and process referrals for home health services
- Coordinate with healthcare professionals, including physicians, nurses, and therapists, to gather necessary patient information and ensure timely initiation of services
- Maintain accurate and up-to-date patient records, ensuring compliance with regulatory guidelines. Collect and verify patient information, insurance details, and medical history
- Communicate effectively and provide excellent service to patients and their families, answer questions, and provide necessary information.
- Monitor and track the progress of patients throughout their stay, and coordinate any necessary adjustments to the care plan.
- Provide administrative support to the clinical team, including data entry, filing, and document management.
- Participate in regular team meetings and training sessions to stay informed about industry best practices and company policies.
Qualifications:
Associates Degree
Experience in Home Health
Excellent communication and interpersonal abilities.
Strong organizational and communication skills.
Ability to prioritize and multitask in a fast-paced environment.
Proficient computer skills, including electronic health record (EHR) systems.
Compassionate, customer-focused approach
Proficiency in Microsoft Office (Word, Excel, & Powerpoint)
Knowledge of home health regulations and documentation requirements.
