Job Summary
We are seeking a dedicated and compassionate Mobile Wound Care Nurse Practitioner to join our healthcare team. The ideal candidate will be responsible for providing specialized care and treatment for patients with acute and/or chronic wounds in various settings, including patients' homes, assisted living facilities, and other healthcare environments. This role involves assessing, planning, implementing, and evaluating wound care treatments to promote healing and prevent complications. Wound Care Experience is vital, as the Mobile Wound Care Specialist will work closely with a multidisciplinary team to ensure comprehensive patient care. This position requires a strong clinical background, excellent communication skills, a commitment to providing high-quality patient care, and travel to patient homes in and around Vero Beach, FL. Service area would include Florida's East Coast from Melbourne in the north to Fort Pierce in the south.
Duties
- Conduct thorough assessments of patients' wounds and overall health status, including history taking, physical exam, and diagnostic tests in various settings.
- Develop individualized care plans based on assessment findings and best practices in wound management.
- Administer appropriate wound care treatments, including dressing changes, debridement, infection control measures, and use of advanced wound care treatments and products as appropriate.
- Educate patients and their families on wound care techniques, self-care practices, and preventive measures to promote healing.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Monitor patient progress and adjust treatment plans as necessary to optimize healing outcomes.
- Maintain accurate documentation of patient assessments, interventions, and outcomes in compliance with healthcare regulations.
- Stay current with advancements in wound care practices and participate in ongoing professional development.
Qualifications
- Current Nurse Practitioner license in the state of Florida.
- Certification in Wound Care is highly preferred, but not required.
- Previous experience in wound care or related nursing specialties is necessary.
- Strong clinical assessment skills with attention to detail.
- Excellent communication and interpersonal skills to effectively educate patients and collaborate with healthcare teams.
- Ability to work independently as well as part of a team in a fast-paced environment.
- Knowledge of infection control practices and wound healing principles is essential.
- Ability to travel to various locations and provide care in various settings.
