Job Description:
Summary:
- A minimum of 1 year of recent experience in a Long-Term care setting
- Active, current LPN license in applicable state or reciprocation
- Full-time LPN experience in a Skilled Nursing facility
Responsibilities:
- Provide direct patient care services, including administering medications, performing treatments, and maintaining accurate records
- Collaborate with healthcare teams to develop and implement individualized care plans that promote optimal patient outcomes
- Conduct thorough assessments and evaluations to identify patient needs and provide appropriate interventions
Requirements:
- A degree from an accredited school of Nursing
- 1 years' full-time LPN experience in a Skilled Nursing facility
