Job Role Summary
Works closely with the Bureau of Environmental Health/Housing & Neighborhood Health Department and other community service providers to maximize health outcomes for individuals, families, and communities. The majority of services are home-based. Responsible for providing a continuum of social work services which will assist patients/clients and their families to find practical ways of resolving social, emotional and environmental problems that interfere with optimum functioning and attainment of total health.
Associated Job Duties
- Provides therapeutic social work intervention to a diverse client system which may include but not limited to psychosocial assessment, short/long term counseling, crisis intervention, community resource referrals, client advocacy, support, outreach, follow-up, and intra/inter agency consultation, specifically in conjunction with the Bureau of Environmental Health
- Initiates social work plans for patients/clients through professional collaboration with health care personnel and other external agencies, utilizing screening questionnaires, diagnostic assessment and individualized treatment plans
- Provides services to patients/clients who demonstrate a need for assistance with psychosocial/emotional/mental health issues, health care, adult/child abuse protection, domestic violence, health insurance, food, clothing, dental assistance, with primary focus being on housing/environmental/shelter problems, impacting their ability to have and or maintain a healthy lifestyle
- Provides direction in the selection, analysis, summarization and presentation of patient/client and related information for service delivery planning, case conferences and professional consultation as it relates to housing and neighborhood health issues and other psychosocial issues, environmental, economic, etc...
- Facilitates the coordination and implementation of services which will assure appropriate services and continuity of care for patients, clients, and their families of Marion County, who have specific housing and environmental related concerns
- Prepares written reports and clinical records (psychosocial assessments, progress notes, summaries, statistical forms, etc.) of services rendered; responds timely to requests for information
- Assumes advocacy/liaison/collaborative role for clients/patients via contacts with community agencies/personnel, hospital personnel, court disposition hearings, abuse cases, etc; collaborates with inter/intra agency personnel in referral activities
- Maintains a current file of referral sources; cultivates a network of resources and collaborates with inter/intra agency personnel involving referral activities
- Patients/Clients and their families’ records and information, staff records of all other confidential information presented orally in writing will be kept confidential
- Maintain confidentiality of patient/client records and information in accordance with HIPAA guidelines
- Assumes responsibility for maintaining current knowledge of information/counseling techniques relevant to specialty area

- Functions as a skilled consultant and advocate, concerning effective management of interpersonal situations which affect the positive coping skills of individuals, families, as it relates issues including housing; and involving active collaboration within a multidisciplinary team
- Documents services rendered via client records, statistical records, and written summaries
- Observes and Interprets impact on client functioning when affected by housing and environmental conditions through home visits, office visits and community encounters
- Assists the Administrator of Social Work and Clinical Coordinator in service and program design, and implementation of social work objectives in regard to housing and neighborhood health and other related services
- Utilizes communication and problem-solving knowledge/skills to identify and facilitate resolution of problems as it relates to psychosocial, environmental, and housing issues
- Participates in staff meetings, in-services, educational programs, and opportunities to enhance knowledge and skills; and assumes responsibility for continuous professional self-development
- Participates on SW/ MCPHD & H&HC/Community committees/projects as required or assigned
- Conducts and/or assists in supervising/orientation of departmental social work students/interns; new and or existing MCPHD/H&HC personnel, to ensure appropriate learning experiences as needed or assigned
- Frequent external contacts with patients/clients, families, health, and social service agencies concerning referrals, care coordination and acquisition/provision of confidential information
- Occasional requirement to testify in court cases (abuses/neglect, custody)
- Serve on appropriate committees and task forces as assigned
- Trained at the appropriate National Incident Management System (NIMS) and Incident Command System
- Training on Psychological First Aid (PSA) in preparation to participate in the response to public health disasters/emergencies
- Participate in the response to public health emergencies, as directed
- Participate in Health Fairs
Qualifications
- Master’s degree in social work (MSW)
- Must be a Licensed Social Worker (LSW)
- Obtain LCSW license when eligible
- Must have a valid Indiana driver’s license

Knowledge, Skills & Abilities
- Graduation with master’s in social work degree from an accredited School of Social Work
- 2-3 years of social work/counseling experience in a community setting is preferred
- Proficient in using Microsoft Office programs and able to learn different EHR systems and computer programs
- Knowledge of pertinent State/Federal laws and regulations, community systems and resources
- Ability to work cooperatively with multi-disciplinary teams in situations requiring a high degree of flexibility, creativity, and initiative
- Apply social work/counseling techniques/principles including evaluative, intervention, counseling methods; toward productive problem solving in the client/worker relationship
- Communicate effectively with a wide variety of people and perform duties independently in a wide variety of settings
- Knowledge and use of computer and skills for writing/documenting case notes, reports, e-mails, etc...
Working Environment
- Possible exposure to communicable diseases, infections, and other hazards, involved in home, office, clinics, and or community site visitations
- Effective response to stress situations; and appropriate response in dealing with difficult patients/clients/families
- In county travel: Visits to office, community locations, and client’s home throughout the county
- Daily contacts with patients/clients, health, social work, and other professionals to initiate/provide/obtain services for patients/clients
- Frequent external contacts with patients/clients, families, health care and social service agencies concerning referrals, care coordination; and acquisition/provision of confidential information
- Occasionally required to testify in court cases (abuse/neglect, custody, environmental issues)
- Patient/Client medical records cannot be transported home or to another worksite without permission from the supervisor and in compliance with MCPHD/HIPPAA guidelines and protocol
- All Patient/Client records and other client information must be stored in a secured locked cabinet in a safe and confidential environment
- Flexibility and high level of organizational skills to respond effectively to changing situations/priorities/locations
