Job Overview

The Health and Welfare Council of Long Island (HWCLI) is a leading health and human services organization dedicated to improving the well-being of vulnerable individuals and families on Long Island. We are seeking a skilled Healthcare Advocate Coordinator to join our team in addressing the complex social determinants of health.

Key Responsibilities

  1. -Verify demographic information and client eligibility using various documentation platforms and systems.
  2. -Collaborate with clients to determine their desire for social care services and document consent accordingly.
  3. -Utilize outreach methods such as virtual, telephonic, or in-person interactions to establish resource needs, connect clients to relevant services, and monitor progress.
  4. -Effectively navigate referrals through Unite Us, ensuring seamless connections to healthcare and social care services.
  5. -Develop tailored care plans based on client eligibility and identified needs, adhering to established timeframes and standards.
  6. -Maintain accurate records of referral status and progress, ensuring timely service delivery and monitoring.

Requirements and Qualifications

  • A high school diploma or equivalent is required, with 2-3 years of relevant work experience preferably in community healthcare settings.
  • -Excellent communication skills, both verbal and written, with the ability to effectively interact with diverse populations.
  • -Proficiency in computer software, including office applications and internet navigation, with experience in MS Office being advantageous.
  • -Knowledge of state and federal benefits systems, with the ability to adapt strategy or approach in response to situational demands.
