Program Overview : The Physical Health Program for Youth aims to improve the physical,
nutritional, social, emotional, and mental health of children, youth, and families in targeted areas
of Orange County, Florida. The program incorporates evidence-based models such as Let's
Move, CDC's Healthy Weight, Nutrition, and Physical Activity guidelines, and Mental Health
First Aid principles.
Position Overview : The Program Manager will lead the implementation of this Program,
providing individualized care plans, education, and resources to participants. Working primarily
in the32805, 32808, and 32811 zip codes, the program will serve 75 unduplicated active
participants annually, delivering 1,000 hours of service through one-on-one interactions, group
settings, and telecommunication options. This role is critical in addressing health disparities and
promoting positive lifestyle changes in underserved communities. Reporting to the Executive
Medical Director, the Program Manager will work collaboratively to ensure the success of this
initiative.
Key Responsibilities :
1. Develop and implement a structured curriculum for program participants focusing on
physical health, nutrition, and mental well-being.
2. Conduct assessments and create individualized care plans for program participants and their
families.
3. Provide nutritional guidance and physical activity recommendations based on evidence-based
models such as Let's Move and CDC guidelines.
4. Organize and lead group presentations and activities to promote healthy lifestyles.

5. Conduct home visits as needed to support participants in achieving their health goals.
6. Maintain records of assessments and interventions using the organization's electronic health
records (EHR) system and other designated tools.
7. Create and maintain a referral resource library to connect program participants with
additional resources and support services.
8. Engage in community outreach activities to promote the program and recruit participants.
9. Collaborate with HCCH / OBFH healthcare providers to identify and engage potential
program participants.
10. Provide educational sessions to participants to enhance their understanding of physical
health, nutrition, and mental well-being.
11. Collect and analyze data for program evaluation and reporting.
12. Coordinate with other HCCH / OBFH programs and community partners to ensure
comprehensive care coordination.
Qualifications :
- Degree in Public Health, Social Work, Nutrition, or related field; Community Health Worker
or Social Worker certification / experience preferred.
