What are the responsibilities and job description for the Occupational Therapist position at Health Care Connectors?
Summary:
An Occupational Therapist (OT) provides professional services within the scope of occupational therapy practice standards in
collaboration with primary care physicians and other members of the health care team and teaches
and educates patients and their families. OT in the home is performed under the supervision of the
RN case manager, Administrator, SCS. If Nursing has opened the case, Occupational Therapy is a
secondary discipline and therefore works collaboratively and professionally with the RN case
manager to provide necessary services
Responsibilities:
- Conduct assessments to evaluate patients' physical, emotional, and cognitive abilities
- Develop individualized treatment plans based on assessment results
- Provide occupational therapy interventions to improve patients' functional abilities
- Monitor and document patients' progress and adjust treatment plans as needed
- Collaborate with other healthcare professionals to coordinate patient care
- Educate patients and their families on therapeutic techniques and strategies
Requirements:
- Master's degree in Occupational Therapy (OT)
- Valid state license or certification as an Occupational Therapist
- Previous experience in case management is preferred
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Excellent problem-solving and critical-thinking abilities
Pay: $70-$80 per hour
