Key Responsibilities:
- Conduct comprehensive patient evaluations to develop individualized treatment plans.
- Implement evidence-based physical therapy interventions to improve mobility and relieve pain.
- Monitor and document patient progress, making necessary adjustments to treatment plans.
- Collaborate with multidisciplinary healthcare teams to ensure holistic patient care.
- Educate patients and caregivers on rehabilitation exercises and at-home care techniques.
- Stay current with advancements in physical therapy to provide the highest quality care.
Qualifications:
- Valid Physical Therapy (PT) license to practice.
- Experience in home health and community care is highly preferred.
- Strong knowledge of medical terminology, anatomy, and physiology.
- Excellent communication and interpersonal skills to engage with patients and healthcare teams.
- Ability to assess patient needs and develop customized treatment plans.
