Overview

The Director of Nursing manages nursing services within the Fallsway clinic and sets the standards for nursing and medical assistant care across all sites. This position collaborates across clinical leadership and in partnership with clients to ensure that excellent care is delivered within the framework of an integrated person-centered health home. This position is a member of the agency Management Team.

Key Role Responsibilities

  • Directly supervises the Fallsway Nurse Manager, Lead CMA,HIV / HCV Care Advocate, and supportive housing Nurse staff. Trains and coaches staff, sets clear expectations, monitors outcomes, creates a culture of open communication and helps team members solve complex problems through individual supervision and team meetings. Fosters a collaborative, supportive and collegial environment to ensure a high-quality experience for clients and high staff morale.
  • Responsible for the standard of nursing care across the agency, providing either direct or indirect clinical supervision for all nurse managers, nurses and medical assistants. Develops RN and MA responsibilities and competencies. Ensures these staff have appropriate education and assessments on principles of care management, chronic disease management, medication adherence, racial equity and inclusion, adult learning, building care plans and interdisciplinary collaboration.
  • In partnership with other clinical and operational directors, responsible for smooth and effective functioning of Fallsway Adult, Family and Triage clinics. Maintains positive, collaborative relationships with providers and practice operations professionals to continuously improve client access, provider efficiency and care coordination. Standardizes processes that simplify workflow and result in coordinated care delivery. Partners to create and enforce clear standards of practice, policies and procedures. Supports a culture of continuous improvement.
  • Collaborates in goal setting during strategic and operational planning and budgeting. Analyzes and presents data for performance measurement, reporting and decision-making. Disaggregates data and sets goals to reduce racial disparities in care. Creates action plans to improve processes. Is responsible for meeting departmental key performance indicators. Implements best practice standards to continually improve quality, safety, customer service and revenue cycle processes.
  • Develops own REI lens and leads an REI approach across nursing services. Commits to continuous self-education to improve the agency’s provision of equitable and inclusive care. Develops trainings, policies and procedures that support the advancement of agency REI principles among staff and clients. Works to identify and reduce health disparities of clients at population and individual level.
  • Helps to create and ensure that staff comply with clinical quality policies and standards that are required by accrediting bodies and federal, state and city government. Provides guidance on proper quality control, storage and handling of onsite medications, vaccinations and point of care testing across all sites.
  • Works with clinical leadership and Quality Assurance to develop and make available patient education materials throughout clinics.
  • Works collaboratively with Directors of Facilities and Compliance departments to provide consultation to Practice Managers for infection control practices at all sites.
  • Provides direct patient care approximately 20% of the time in accordance with the Registered Nurse job description.

  • Protects clients’ personal health information by maintaining compliance with HIPAA and other relevant health care-related IT security regulations
  • Performs other duties on an as-needed basis
  • Knowledge, Experience and Skills

    Formal Education and Training

  • Possess current licensure as a Registered Nurse in Maryland
  • Advanced degree in nursing or health administration preferred
  • BSN required
  • BLS CPR certification
  • Experience

  • Five years of experience as an RN
  • Two years in a community health center (FQHC) or primary care facility. FQHC experience is highly preferred.
  • Two years of direct supervision of nursing staff
  • Experience working with underserved populations
  • Experience working collaboratively in a multi-disciplinary clinical setting highly preferred
  • Applied knowledge of trauma-informed care and harm-reduction philosophy and practices
  • Skills
