Overview
The Director of Outreach Services is responsible for overseeing and managing the outreach programs at the organization, including the mobile clinic, street medicine, outreach team, and community partnerships. This role is critical in ensuring that underserved and vulnerable populations receive access to high-quality healthcare services. The Director will develop and implement strategies to expand outreach efforts, enhance community engagement, and improve health outcomes. This role will also provide clinical services at one of the Agency’s sites. This position is a member of the agency’s Management Team.
Key Role Responsibilities
- Oversee the daily operations of the agency’s outreach programs, including the mobile clinic, outreach team, and street medicine program. Provide Leadership, supervision, and support to all outreach staff, peer recovery specialists, ensuring high-quality service delivery, and develop and implement strategic plans to enhance outreach services and expand program reach.
- Creates a positive and collaborative team environment by working with an interdisciplinary team of healthcare providers, nurses, social workers, peers, administrative and outreach staff. Promote open communication, mutual respect, and shared goals to ensure cohesive and effective delivery of service.
- Design and implement outreach strategies centering the client in their care informed by harm reduction and trauma informed care principles to engage and serve homeless populations in Baltimore City and County. Collaborate with healthcare providers, social workers, and community partners to ensure comprehensive care. Additionally, monitor and evaluate program effectiveness, adjusting as needed to improve outcomes.
- Responsible for identifying outreach locations, prioritized areas in Baltimore City and County for agency outreach engagement. In partnership with other departments, leads annual needs assessments and identifies areas of opportunities and incorporates results into annual outreach strategies for the agency.
- Establish and maintain strong relationships with community organizations, government agencies, and other stakeholders to improve agency presence in the community and support program goals. May represent Health Care for the Homeless at community meetings, events, and forums, advocating for the needs of homeless populations and promoting the organization's mission.
- Actively develops own racial equity and inclusion lens and supports development of REI lens in colleagues. Prioritizes improving access to care through an anti-racist approach. Leads and partners across the agency to address racism and racist practices in our work, including ways to mitigate disparate outcomes in health care delivery.
- Partner with fellow agency leaders to mitigate safety risks for client and staff. Monitor, evaluate and ensure administrative practices and environments of care are following local, state, and federal laws, as well as accreditation and certification guidelines to abate hazards, safeguard clients and staff, and ensure provision of quality health care.
- Serves as a member of a cross-disciplinary clinical leadership team, including medical, operations and behavioral health providers. Guides strategic initiatives, and establishes and meets annual clinical, population health, risk management and budgetary goals. Works with clinic operations leaders to set goals and improve access for clients unable to get to a fixed site.
- Develop and manage program budgets, ensuring financial sustainability. Partner with Sr. Director of Health Informatics to ensure appropriate data collection and reporting to track program outcomes and impact for agency and grant requirements. Prepare reports and presentations for the Management Team and Board of Directors.
- Coordinates with IT and Facilities Departments to ensure optimal performance of information systems, communication systems and other technological needs.
- Maintains a small client panel to whom clinical services are provided up to four sessions a week.
