Overview

The MAT RN provides care coordination for clients participating in Health Care for the Homeless’ Medication-Assisted Treatment Stabilization program and the agency-wide Syringe Service program. The MAT RN is responsible for the coordination of MAT initiation for new clients, Buprenorphine prescriptions, syringe services and distribution of safer drug use materials, community outreach, and assisting with the implementation of overdose prevention / Naloxone training and distribution. The MAT RN may spend time working at the Fallsway, County, or mobile locations.

Key Role Responsibilities

  • Assist with the coordination of Health Care for the Homeless’ MAT program including maintaining a roster of MAT participants and coordinating insurance prior authorizations.
  • Coordinates care of clients within the agency, including assisting clients in navigating care team and non-care team appointments; addressing issues that arise outside scheduled appointment times; monitoring client adherence to primary and psychiatric care; and referral to behavioral health providers.
  • Delivers health education and counseling, drawing upon the individual’s strengths, and motivation. This may include delivering interventions that recognize the impact of trauma and utilizing clinical approaches such as harm reduction and motivational interviewing to support the individual’s wellbeing, improve health care utilization, and reduce risk behaviors.
  • Serves as a liaison to community health workers, peer recovery specialists, and assists clients in building individualized support systems.
  • Perform clinical tasks including triage of walk-in clients, client education, review of preventive health needs and nursing assessments.
  • Work in accord with the Baltimore City and Maryland Health Departments and other community partners on strengthening compassionate overdose prevention strategies, decreasing barriers to care, and integrating harm reduction into healthcare.
  • Practice collaboratively within the multidisciplinary team and within the scope of RN licensure, providing nursing support and community linkages.

  • Knowledge, Experience and Skills

    Formal Education and Training

  • BSN required
  • Possess current licensure as a registered nurse in Maryland
  • BLS CPR certification
  • Experience

  • Two to three years of direct client experience working in a community health setting
  • Experience in harm reduction training and advocacy or as a public health RN preferred
  • Experience working collaboratively in a multi-disciplinary clinical setting
  • Skills

  • Strong interpersonal skills necessary to work with vulnerable populations and to collaborate with other team members, other staff members, and personnel of other agencies
  • Possess the ability to make independent decisions when circumstances warrant such action
  • Able to cope with interruptions, be flexible and be a team player
  • Able to work with ill, disabled, emotionally upset and sometimes hostile individuals
