Responsibilities :
- Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
- Obtain FCT Certification within one year of employment to continue providing FCT services and maintain employment.
- Complete FCT Checkoffs and internal trainings within designated deadlines monitored through an e-learning system.
- Deliver FCT services to children, adolescents, and their families, primarily in their living environments, ensuring a family-focused approach.
- Provide 24 / 7 on-call coverage to a caseload of families, requiring a flexible work schedule to accommodate family needs.
- Complete all required Clinical Records and FCT documents and ensure timely entry / upload into the Electronic Health Record according to standards.
- Develop and maintain Service Plans collaboratively with the Treatment Team, ensuring coordinated communication and service provision.
- Conduct ongoing revisions of Service Plans and implement necessary strategies.
- Provide psychoeducation as outlined in the Service Plan.
- Facilitate interventions aligned with the FCT model, guiding families through the four phases of treatment.
- Assist families in establishing and achieving weekly goals.
- Make referrals to additional services and resources for families and children.
- Develop and coordinate discharge plans based on treatment needs and community resources.
- Oversee initial and ongoing assessment activities.
- Engage with Treatment Team members for Service Plan coordination.
- Consult with external providers to ensure comprehensive treatment and community support.
- Monitor and document the consumer's progress regularly.
- Participate in Quality Assurance / Quality Improvement activities including audits and improvement initiatives.
