- Schedule: Monday, Tuesday, Thursday, Friday 8a-5p and Wednesday 10a7p, Outpatient
- Patients per day: 18-21
- Practice Setting: Federally Qualified Health Center
- State Licensure: Connecticut
- Responsible for Mid-levels
