- Coordinate the transfer of patients to and from the hospital, working closely with physicians, nurses, and other healthcare professionals.
- Assess patient information and clinical data to facilitate appropriate bed assignments and prioritize transfers based on clinical needs.
- Communicate effectively with referring and receiving departments to ensure timely and efficient patient transfers.
- Maintain accurate records of all transfer requests and patient movements within the hospital’s electronic health record system.
- Participate in the development and implementation of policies and procedures to improve the efficiency of the Patient Logist
