Responsibilities

  • Facilitate interdisciplinary team for patient care planning and evaluation.
  • Coordinate care across the continuum, ensuring optimal patient outcomes.
  • Conduct utilization reviews and collaborate with payers.
  • Implement quality improvement strategies and assess educational needs.
  • Compile and report data on patient care services.

Qualifications

  • Registered nurse with a current license.
  • Bachelor's degree required; Master's preferred.
  • Minimum three years clinical experience; case management experience preferred.
  • Effective communication and problem-solving skills.
