Summary of Duties
As a Community Outreach Specialist, you will lead efforts to connect underserved community members with healthcare services, health insurance enrollment, and other vital resources. Your work will directly contribute to improving the health and well-being of individuals and families.
Essential Responsibilities

Community Outreach and Collaboration:
- Plan, coordinate, and prepare for outreach events to reach residents without access to healthcare or health insurance.
- Build and strengthen enrollment collaborations with local partner organizations.
- Educate the community on medical outreach and educational programs.
- Healthcare and Resource Assistance:
- Provide health insurance enrollment and application assistance.
- Assist patients in completing sliding fee schedule applications and ensure documentation is accurate and complete.
- Help patients access necessary resources, including food, shelter, clothing, and transportation.
- Provide medication assistance through partnering pharmacies and prescription programs.
- Administrative Support:
- Assist with the preparation and gathering of various reports.
- Participate in community activities to heighten health awareness.
- Ensure accurate and thorough documentation of all interactions and assistance provided.
- Additional Duties:
- Arrange transportation as needed for families to access healthcare.
- Perform other duties as assigned by the supervisor or management.

Requirements:EDUCATION/EXPERIENCE: A degree in public health, health education, communications, or related program preferred; Bilingual preferred (English/Spanish); The ability and willingness to interact with others in both a small and large group setting is essential. Must possess a true desire to help others and display compassion and empathy in day-to-day activities. Strong computer skills with proficiencies in Outlook, Word, PowerPoint, Excel, and internet-based applications. On-the-job training provided.
