Job Overview
Health Force is a family owned and operated home health care agency. We are accredited with distinction by the Commission on Accreditation for Home Care and have consistently provided the highest level of quality non-medical home health aide services to residents of Mercer County for the past 28 years.
We are currently seeking a dedicated Registered Nurse to manage our home health clinical services. This is a part time position (minimum 25 hours per week) leading to full time based on patient census.
Job Description:
As the Director of Patient Care Services you will have the responsibility of overseeing the delivery of all clinical services provided by the agency. Primary responsibilities include:
Perform client initial and 60 day assessments
Develop client care plans
Assure care plans are complete, accurate, followed by the direct care staff and continue
to meet the clients’ needs
Communicate with clients’ physicians
Coordinate client care with clients’ healthcare team
Serve as the direct clinical supervisor for the field nurses and home health aides
Review progress notes
Ensure the clinical records are in compliance with state and Medicaid regulations
Perform quarterly quality assurance record reviews
Work closely with the owner(s) in assessing the needs of the overall business
Rotating on-call phone responsibility
This position includes both field and administrative functions of nursing. The field duties include physical assessments, visiting with families, coordination with the interdisciplinary team and supervision of the home health aides. Administrative functions include ensuring compliance with regulatory requirements regarding both the delivery of care and staff/client files.
Qualifications:
. Must possess a current, valid license in good standing as a registered nurse issued by the New Jersey Board of Nursing.
. A Bachelors of Science in Nursing (BSN) preferred with at least two (2) years of nursing
experience in community and/or home health is required or an RN with three (3) years
of experience in community and/or home health is required.
. Experience must include providing nursing services in a patient’s home environment
inclusive of an assisted living facility.
. Prior supervisory/management experience preferred
. Strong interpersonal and communication skills are desired
. Must be organized, detail oriented, self motivated and possess the ability to multitask
. Knowledge of Medicaid regulations a plus
