Job Role Summary
The Sandra Eskenazi Mental Health Center Employment Consultant provides assistance to individuals with physical and / or psychiatric disabilities in obtaining and retaining competitive work in a community setting. This position is responsible for orienting, training, coaching and supporting individuals to make placements successful.
Essential Functions and Responsibilities
- Provides assistance to individuals with physical and / or psychiatric disabilities in obtaining and retaining competitive work in a community setting
- Responsible for orienting, training, coaching and supporting individuals to make placements successful
- Completes intake and assessment to determine optimal employment opportunity for each individual, including assessment for vocational interest, support systems, past / present problem areas, assets / potentials, skill and needs
- Formulates a vocational rehabilitation plan in collaboration with the individual and their primary Clinician / Care Coordinators and others as requested by the individual.
- Participates in interdisciplinary meetings and scheduled conferences providing appropriate input
- Carries out job matching / development through comprehensive search in community for potential employment sources which includes making contacts with potential employers and developing ongoing working relationship with them
- Develops job specifically to match patient needs
- Completes all required client / service activity forms to ensure accurate information for client billing, setting fees and management information according to the Center policies and procedures
- Completes FSSA documentation and record keeping as required
- Meets the agency standards of timely completion of all documentation
- Meets the agency standards of service hours
