EPIC experience and experience in the EPIC Dyad model a big help.
Job Requirements
Inpatient case management experience with discharge planning and pre-certification UM experience.

Day Shift, 40 hours guaranteed.

Additional Details
BLS required.
Radius Rule : 50 miles AND candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and / or Dignity.
Driver's license required at submittal and will be used to determine if candidate is outside 50 miles.
