Job Summary:
We are seeking a dedicated and compassionate RN to join our hospice team as a case manager. In this role, you will coordinate care with healthcare professionals, evaluate the effectiveness of services, and consult with physicians on care changes.
Key Responsibilities:
- Coordinate hospice care with interdisciplinary teams and families.
- Evaluate the effectiveness of services and consult with physicians on care adjustments.
- Submit timely clinical notes and patient schedules.
- Participate in IDG and case conferences.
- Conduct comprehensive patient assessments and supervise Hospice Aide care.
- Maintain current care plans and perform on-call duties.
Qualifications:
- RN license required; graduate of an accredited nursing school.
- Experience in long-term care, homecare, or hospice preferred.
- Strong communication and problem-solving skills.
- Pass background checks and drug screening.
